Grant Request Form

REQUIRED DOCUMENTS:

• Attach a copy of price quote (if available)

• Documents illustrating use or justification of request

NOTE: 3 BIDS MUST BE PROVIDED IF REQUEST EXCEEDS $1,000.00

All requests will be need to have electronic approval prior to the request being placed on the agenda. Requesting Coach is required to be at the meeting in which the topic will be discussed.

STEP 1: Upload completed form and additional documents to Athletic Director for approval.

STEP 2:  Athletic Director emails approved/denied grant request form to Boosters President prior to General Meeting. 

STEP 3:  Processed forms will be submitted to Treasurer and Secretary.
